Medical, Vision, and Dental Benefits (Pre-tax Premium)
3H contributes 80% of the cost of Medical and/or Dental Insurance toward any plan you select. Coverage begins on date of hire and is available to full-time employees and their dependents.
- Anthem PPO for Medical.
- Prescription coverage with no deductible and 3 co-pay tiers of $8/$15/$30.
- Vision plans provide coverage toward an annual exam, lenses and frames, or contact lenses.
- The Dental plan, Guardian DentalGuard Preferred PPO, includes preventive care, basic and major dental services
Life Insurance, STD, LTD, and AD&D Benefits
3H provides Life Insurance benefits at no cost to the employee.
- Life Insurance: 200% of employee’s annual salary to maximum of $400,000.
- Short-term Disability: 60% of employee’s weekly salary up to a weekly maximum of $1,500 after a 8 day elimination period for maximum of 12 weeks of benefits.
- Long-term Disability: 60% of employee’s monthly salary up to a maximum of $10,000 per month after 90 day elimination period.
- Accidental Death & Dismemberment ( AD & D) insurance equals the Life Insurance in effect at the time of the loss.
Voluntary Term Life Insurance for Employees and Dependents
Employee paid, optional coverage.
- Employee may elect an amount in increments of $5,000, not to exceed the lesser of 5 times basic annual earnings or $300,000. Employees may elect up to $100,000 in coverage without Evidence of insurability.
- Spouse Life: Increments of $5,000, not to exceed 50% of the employee’s Life amount, subject to a minimum of $10,000 and a maximum of $50,000. Spouse may elect up to $25,000 without Evidence of insurability.
- Child life insurance: $5,000.
401(k) Plan (Pre-tax)
Participation begins the first of the month following your date of employment.
- 100% match on deferrals up to 2% and 50% match on deferral on the next 3%.

Education Assistance
- Available after 12 months of employment for employees in satisfactory performance standing.
- Reimbursement of up to $5,250 of tuition expenses for courses at an accredited college or university per calendar year.
Employee Assistance Program (EAP) INOVA Healthcare
EAP benefits include 24/7 telephone counselor availability with multilingual and TDD services, 4 face-to-face counselor meetings, assessment and counseling for drug, alcohol and substance abuse, wellness newsletters and employee seminars.
Flexible Spending Plan (Pre-tax)
- Medical Flexible Spending (up to $3,000 per year).
- Dependent Care (up to $5,000 per year).

Transportation Plan (Pre-tax)
- Metro Check and SmarTrip – Employee may set aside up to $105 per month pre-tax to purchase Metro Checks through payroll deductions.
- Parking Plan – Employee may set aside up to $205 per month pre-tax to purchase parking permits at certain lots near their work location or at Metro Parking Lots.
