Medical, Vision, and Dental Benefits (Pre-tax Premium)
Coverage begins on the first of the month following your date of hire and is available to full-time employees and their dependents, including domestic partners.
- Medical Plans - Choice of 3 Health Plans from Anthem BCBS
- Prescription coverage is included with the Medical Plans, with no deductible and 3 co-pay tiers
- Vision Plans - provide coverage toward an annual exam, glasses and contact lenses. Over 24,000 VSP network doctors nationwide
- Dental Plans - 2 Options from Delta Dental. Both options include coverage for preventive care, basic and major dental services.
Life Insurance, STD, LTD, and AD&D Benefits
3H provides these benefits at no cost to the employee- Life Insurance: 2X annual base pay (maximum $600,000)
- Basic AD&D: 2X annual base pay (maximum $600,000)
- Short-term Disability: 70% of employee’s base salary up to a weekly maximum of $2,500 after a 8 day elimination period, for maximum of 12 weeks of benefits
Supplemental Term Life Insurance for Employees and Dependents
Employee paid, optional coverage.
- Can elect coverage in $10,000 increments
- Maximum amount is the lesser of $600,000 or 5X annual salary
401(k) Plan (Pre-tax)

Education Assistance
